THE RECIPIENT BOSTON TERRIER RESCUE GROUP OR PERSON RESERVES THE RIGHT TO REFUSE ANY APPLICATION

By submitting this form I/We certify that all of the information on this application is true and complete. I/We understand that failure to provide truthful information will render this application null and void and will result in the immediate disqualification of my/our application.

I/We understand that if selected as a foster home I/We will comply with the following conditions:

Upon approval of this application, FOSTER shall enter into a FOSTER AGREEMENT with BOSTON TERRIER RESCUE OF FLORIDA ("BTRFL") setting forth terms and conditions that substantially are similar but not limited to the conditions set forth in this application.

FOSTER acknowledges that BTRFL depends on foster homes to provide a safe and loving home for the foster dog(s) while they find their permanent homes. Therefore, FOSTER assures that FOSTER’s true and only intent in completing this application is to become a FOSTER for BTRFL rather than to adopt. If, in the future, FOSTER decides to adopt the foster dog(s), FOSTER agrees to make a good faith effort to continue fostering with BTRFL.

FOSTER shall keep FOSTER BOSTON as an indoor pet and will provide BOSTON with identification at all times (i.e., collar tag, tattoo, or microchip). BOSTON will be kept under control at all times when outside FOSTER'S property.

FOSTER agrees to use a veterinary provider approved by BTRFL and not to exceed the pre-approved costs for initial vet care. The Medical Director will assist FOSTER in determining customary and reasonable vet expenses. The rescue expects and demands high performance, a high skill level, substantial experience and reasonable costs from the vets it uses.

FOSTER agrees to provide the foster dog(s) with adequate companionship, food, water, grooming, exercise, as well as regular and customary health care as instructed by BTRFL. Moreover, All foster homes and volunteers shall act in the best interest of all rescued/fostered dog(s).

FOSTER agrees to notify BTRFL of any change of address or telephone number of FOSTER.

If BOSTON is lost or stolen, FOSTER will immediately notify BTRFL.

FOSTER agrees to keep personal pets and children quarantined from the rescued dog(s) for the first two to four days minimum.

FOSTER agrees and understands that BTRFL, makes no warranties or representations as to the health, condition, quality, temperament or suitability of the animal placed under the terms of Foster and Volunteer Agreement. It is agreed and understood by FOSTER that while every effort is made to provide accurate history and assessment of a dog, BTRFL does not warrant a dog regarding medical status or behavior or disposition. It is further agreed that environmental changes may effect and change the temperament of the dog and that BTRFL has no liability or responsibility of any nature regarding later defects with the dog, or injuries or damage to any person or property which may be caused by the dog. Furthermore, FOSTER acknowledges that fostering involves risks resulting from potentially aggressive or fearful dogs, dogs that have been abused, and dogs that have been relinquished or abandoned due to serious heath concerns or pregnant or nursing females. All foster homes are expected to carry healthcare insurance on themselves and family members. FOSTER agrees to release, indemnify and hold harmless BTRFL and its members or representatives against any and all claims for any injury or property damage to the Foster Home or anyone else.

FOSTER shall return BOSTON to BOSTON TERRIER RESCUE if, under any circumstances, FOSTER is no longer able to keep or provide for BOSTON. FOSTER shall not sell, give or otherwise transfer or convey BOSTON to anyone other than BOSTON TERRIER RESCUE.
